If you are not finding the patient you need, you might try casting a broader search - yes, we like to be as precise as we can.  But, with SQL tables, sometimes we need to ask a question which will return a larger subset of data.  For example,  Looking for Mary C Smith with a DOB of 1/10/1999,  If there are several Mary Smith with same DOB, you may or may not get the one that you want!   

I had exactly this problem.  E.g.  Mary C Smith and  Mary J Smith both had same DOB but were two different people.  Entering Mary Smith and Mary J Smith got the wrong person and entering Mary C Smith got the same wrong person!  Entering phone number and address got me NO RESULT!   A call to my local PDMP Admin and after a few questions the answer was to use a PARTIAL SEARCH to find a list of ALL the patients in order to display the one I wanted and then be able to choose that one.
